At Root & Restore Wellness, we offer three levels of membership based on your health needs and the level of care you may require. This allows us to provide personalized care while ensuring every patient receives the time and attention they deserve.

ESSENTIAL CARE

$89/Month

For generally healthy individuals or those with 1–2 stable conditions.

This tier is designed for patients who are generally healthy or have one to two stable chronic conditions that require routine monitoring.

Examples may include:

• Mild hypertension

•Well-controlled thyroid disease

• Mild anxiety or depression

• Routine preventive care needs.

This membership includes access to primary care services such as preventive care, sick visits, and routine health management.

COMPREHENSIVE CARE

$139/Month

For individuals managing multiple chronic conditions that require closer monitoring.

This tier is designed for individuals managing three to four chronic health conditions that are generally stable but require more frequent monitoring and coordination of care.

Examples may include:

• hypertension
• diabetes
• thyroid disease
• high cholesterol
• asthma

Patients in this tier typically benefit from more frequent visits and closer management to maintain stability and prevent complications.

COMPLEX CARE

$199/month

For patients with complex or uncontrolled health conditions requiring frequent visits and coordination of care.

This tier is designed for individuals with multiple complex conditions, frequent symptom flares, or conditions that require intensive monitoring and coordination of care.

Examples may include:

• uncontrolled diabetes
• autoimmune disease with frequent flares
• complex metabolic conditions
• multiple chronic illnesses requiring close follow-up

This membership allows for more frequent visits, care coordination, and ongoing support to help stabilize and manage complex health needs.
